These forms streamline the billing process by allowing practices to customize layouts for Medicare, standard insurance, or text-only formats. With versions tailored for different payer requirements, Lytec forms ensure accurate, professional submission of claims and improve overall workflow efficiency in medical offices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Lytec comes with 3 main types of claim forms:<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"779\" height=\"485\" src=\"https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-2.webp\" alt=\"\" class=\"wp-image-1171\" srcset=\"https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-2.webp 779w, https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-2-300x187.webp 300w, https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-2-768x478.webp 768w\" sizes=\"(max-width: 779px) 100vw, 779px\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The forms with \u201cCMS 1500 02-12\u201d in the name are the new ICD-10 forms.&nbsp; These are the forms typically used on claims after October 2015:<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"269\" height=\"144\" src=\"https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image.png\" alt=\"\" class=\"wp-image-1166\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Next are the \u201cCMS 1500\u201d forms (<em>that do not have \u201c02-12\u201d in the name<\/em>).&nbsp; These are typically not used any more, but may be requested by local\/regional insurances, or other entities:<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"236\" height=\"146\" src=\"https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-2.png\" alt=\"\" class=\"wp-image-1170\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Lastly are the \u201cText-CMS\u201d forms.&nbsp; These are used for generating \u201cprint-image\u201d claims for clearinghouses.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"169\" height=\"81\" src=\"https:\/\/www.2kmedicalbilling.net\/Lytec-Blog\/wp-content\/uploads\/2025\/04\/image-1.png\" alt=\"\" class=\"wp-image-1168\" \/><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Amongst the 3 main types of forms, there are a few more variants.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Forms that have \u201cwith form\u201d in the name are used with blank-paper, and will print the lines, boxes, and numbers\/descriptions for each box.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Forms that have \u201cMedicare\u201d in the name may be used for some Medicare insurances.&nbsp; The biggest difference with these forms is that many of the fields\/boxes in the top-right area of the form will not be populated.&nbsp; Please note however, not all Medicare insurances want these forms.&nbsp; For those carriers, and most others, the \u201cStandard\u201d forms should be used.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Forms that have \u201cStandard\u201d in the name are the most commonly used forms.&nbsp;
